Template — must be reviewed before this page goes live. This is a starting point written to cover what an insurance agency site of this shape normally needs. It is not legal advice and it has not been reviewed by an attorney. Every bracketed field has to be filled in, and the whole page should be checked against the agency's own compliance guidance and the rules of the states it is licensed in. Delete this box once that's done.

Privacy Policy
This policy explains what information we collect through this website, how we use it, and the choices you have. Last updated [date].
Who we are
We are an independent insurance agency. We help individuals, families, and self-employed business owners compare and enroll in health coverage. You can reach us at {{location.email}}, {{location.phone}}, or [mailing address].
Information we collect
- Information you give us directly. Your name, phone number, email address, and anything else you choose to tell us when you book a consultation, submit a form, or contact us.
- Information we collect to quote coverage. If you go on to work with us, we will ask for details needed to find and apply for a plan — date of birth, household size, income range, ZIP code, the doctors and prescriptions you want covered, and health history where a plan requires it.
- Information about someone you refer. If you use our referral form, we collect the name, phone number, email address, and any context you provide about that person, along with your own name and contact details.
- Technical information. Standard web data such as IP address, browser type, pages viewed, and how you arrived at this site.
Health information
Some of what we collect to quote coverage is health-related and more sensitive than the rest. We ask for it only where a carrier or plan requires it, we use it only to find and apply for coverage on your behalf, and we hold it no longer than we need to.
How we use your information
- To respond to you, schedule and hold consultations, and answer questions.
- To compare plans, check your doctors and prescriptions against plan networks and drug lists, and submit applications you ask us to submit.
- To service your coverage afterward — renewals, changes, billing questions, and claim or network problems.
- To meet legal, licensing, and carrier recordkeeping obligations.
Who we share it with
We do not sell your personal information.
We share it only where it's needed to do the work you've asked us to do, or where the law requires it:
- Insurance carriers and their underwriters, when you ask us to quote or submit an application.
- Service providers who run our business — our customer relationship management system, scheduling, email and phone tools, and our marketing organization — under agreements limiting them to working on our behalf.
- Regulators, law enforcement, or others where we're legally required to disclose it.
If someone referred you to us
Our referral form asks the person submitting it to confirm they've already asked you and that you're expecting to hear from us. If we've contacted you and you weren't expecting it, tell us and we'll remove your details straight away — {{location.email}} or {{location.phone}}. You don't have to give a reason.
Calls, texts, and emails
When you give us your phone number, you agree that we may call or text you about health coverage, including with an automatic telephone dialing system or a prerecorded message where you've separately agreed to that. Consent to marketing messages is not a condition of buying anything. Message and data rates may apply.
You can stop marketing texts by replying STOP, stop marketing emails using the unsubscribe link in any of them, or ask us to stop calling by telling us on any call or in writing. We may still contact you about coverage you already have with us.
Cookies and analytics
This site uses cookies and similar technologies to keep it working properly and to understand how it's used. Most browsers let you refuse or delete cookies in their settings; parts of the site may not work as well if you do. [If the agency runs advertising or analytics pixels — for example Google Analytics or the Meta pixel — list them here and say what they do.]
Your choices and rights
You can ask us at any time to:
- Tell you what personal information we hold about you and where it came from.
- Correct anything that's wrong.
- Delete it, where we're not required to keep it.
- Stop contacting you.
Depending on where you live, state privacy law may give you additional rights — including the right to know, to delete, to correct, to opt out of sale or targeted advertising, and not to be treated differently for exercising them. We don't sell personal information or use it for cross-context behavioral advertising. To make a request, email {{location.email}}. We may need to verify your identity first.
How long we keep it
We keep your information for as long as you're a client and afterward for as long as carrier, licensing, and state recordkeeping rules require. When we no longer need it, we delete it or remove anything identifying.
Security
We use reasonable administrative, technical, and physical safeguards to protect the information we hold. No method of transmitting or storing information is completely secure, so we can't guarantee absolute security. Please don't send sensitive health or financial details by unencrypted email.
Children
This site isn't directed at children under 13 and we don't knowingly collect their information. We do collect information about children when a parent or guardian includes them on a family application. If you believe a child has given us information directly, contact us and we'll delete it.
Other sites
This site links to carrier sites and other third parties. We don't control them, and this policy doesn't cover what they do with your information.
Changes to this policy
We may update this policy. When we do, we'll change the date at the top of the page. Material changes will be posted here.
